The aim of this work was to assess of state of the vascular bed and perfusion of lungs in patients with chronic thromboembolic pulmonary hypertension (CTEPH) using the method of subtraction CT.
Methods and Materials
CT pulmonary angiography (CTPA) was performed 45 patients with suspected diagnosis of CTEPH (18 men,
age 26-79 years) by 320-detector row CT scanner (Aquilion ONE 640 VISION Edition,
Toshiba) using the standard protocol Lung substraction.
The main pulmonary artery (MPA),
CT angiographic (CTA) obstruction score,
perfusion defect score,
parameters right ventricular (RV) were assessed by CTPA.

In all patients were detected pulmonary perfusion abnormalities.
Thrombotic masses in pulmonary arteries were identified in 42 patients.
In 3 patients was detected intraluminal bands and webs in segmental arteries by CTPA,
which didn't detect by digital subtraction angiography (DSA).
The mean values of CTA obstruction score and perfusion defect score were 38,1 ± 21,3% and 53,7 ± 21,1%,
The mean value of mean pulmonary arterial pressure (mPAP) was 47,9±12,8.
Subtraction CTPA may be helpful for assessing the severity of thrombotic lesion of the distal vascular bed and an objective assessment of the effectiveness of treatment in patients with CTEPH.
